Substantiation Requirements for Mileage and Parking

New, Easier Process for Mileage and Parking Reimbursement

You spoke, and we listened! You no longer need to submit receipts to be reimbursed for eligible mileage and parking expenses for travel to and from your doctor, dentist, pharmacy, or other medical care provider.

Simply document the required information on the Mileage Worksheet (PDF) (PDF).

You must sign and date the Certification and Authorization Statement before you submit the form. Any Mileage Worksheet submitted without a signature or date will not be reimbursed.
